Create Incident

Creating an incident allows you to track issues that happen at your facility. To create an incident:

  1. Access the Incident module by clicking the Incident link at the top of the page.

  2. Select the Create Incident tab.

  3. In the Property field, select the property where the incident occurred.

  4. In the Space/Floor field, select the space or floor where the incident occurred.

  5. In the Incident Type field, select the kind of incident, for example, fire or medical emergency.

  6. In the Incident SubType field, select a value to further define the incident type.

  7. In the Location field, enter any additional information about the place where the incident occurred.

  8. Enter values in the Incident Date and Time fields to indicate when the incident occurred.

    The system will default to the current date and time. To enter a different value, you can enter the date or click the calendar icon to choose the date.

  9. In the Root Cause field, select the cause of the incident.

  10. Enter values in the Incident End Date and Time fields to record when the incident ended.

  11. In the Other field, enter any additional information about the incident.

    This field is often used to provide a description for the value entered in the External Incident ID field. For example, if the external incident ID references a police report number, you might enter "Police report."

  12. In the External Incident ID field, enter a value for any external incident ID, for example, a police report number.

  13. To indicate a vendor involved in the incident, choose the Company Doing Work by clicking the Select link.

  14. In the Asset field, specify any asset that was involved in the incident. You can click the Select link to select a value from a list of available assets.

  15. In the Confidential field, select Yes to mark the incident as confidential.

    Marking an incident as confidential prevents unauthorized users from viewing the incident. Your security rights determine if you can view confidential incidents.

  16. In the Department field, select a value to indicate the specific department involved in the incident.

  17. In the Incident Summary section, enter a comment to describe the incident in the Incident Description fields.

    The needs of your facility determine how many incident description fields are available.

  18. To record the cause of the incident, enter a description in the Root Cause Investigation field.

  19. In the Incident Reported By fields, enter values to identify the person reporting the incident.

  20. In the Incident Entered By fields, enter values to identify who entered the incident.

  21. Click ADD.
